-Defined 'open-quote', 'close-quote', 'no-open-quote', 'no-close-quote'
and the nesting depth of quotes.
-Inserted definitions of 'counter-reset' and 'counter-increment'.
-Added a rule that a 'compact' element that contains a hard newline
(such as BR) cannot be rendered inline.
