[css-text-3] Editorial: source code reorganization

This commit is editorial, and cleans up the source code of the spec:
* Use tabs for indentation, spaces for alignment
* Use semantic line breaks
* omit optional html tags
* Take advantage of bikeshed's markup and cross linking facilities
* Fix a handful of typos and punctuation inconsistencies
* Use syntax highlighting
