[css-ruby] Modernize markup

* Drop optional HTML tags (e.g. </p>)
* Drop tags that are optional when using bikeshed (most <p>)
* Use bikeshed's "Note:" and "Issue:" notations
* Use bikeshed's syntax highlighting
* Take advantage of bikeshed's handling of spaces inside <pre> tags
